1.  Statement of marks of every year / semester of the qualifying examination.

2.  Transference / Leaving certificate from the educational institution previously attended (in case of candidates passing the qualifying examination from Pune University).
     Applicable to candidates who have passed qualifying examination on or before June 2007.
     Candidates who are appearing for the qualifying examination in the current academic year are NOT required to submit this document along with this form.

3. Migration certificate from the University previously attended (in case of candidates passing qualifying examination from Universities other than Pune University).
    Applicable to candidates who have passed qualifying exam. on or before June 2007.
    Candidates who are appearing for the qualifying exam. in the current year are NOT required to submit this along with this form.

4. Reserved Category Certificate from the competent authority (applicable to candidates belonging to reserved categories of Maharashtra state only).

5. Non-creamy layer certificate if candidate belongs to NT-C, NT-D, or OBC Categories of Maharashtra.

6. Birth certificate / school leaving certificate.

7. Certificate from a competent authority (District Civil Surgeon) showing minimum 40% physical disability for candidates having physical disability.
